2. 將填妥的申請表及所需文件郵寄/WHATSAPP/電郵至本公司。
3. 在接獲填妥的申請表及相關文件後， U-Drive 油卡將在7－10天內郵寄至申請人登記的香港通訊地址。
個人申請
以個人名義申請所需文件：
1. 申請人身份証副本
2. 車輛登記文件 【牌簿】正面副本
3. 地址證明文件副本 (如水/電/煤氣費單或差餉單)
公司名義申請
以公司名義申請所需文件：
1. 公司商業登記証及CI副本
2. 公司持牌人或董事身份证副本
3. 公司持牌人或董事地址證明文件副本 (如水/電/煤氣費或差餉單)
Application
How to apply:
1. Download [Application Form]
2. Mail/WHATSAPP/email the completed application form to our company.
3. After receiving the completed application form and related documents, U-Drive Fuel Card will be mailed to the registered Hong Kong correspondence address of the applicant within 7-10 days.
Individual Applicant
Required documents for individual application:
1. Copy of the applicant's HKID card
2. Copy of the Vehicle Registration Document (front page)
3. Copy of address proof (such as water/electricity/gas or rates bill)
Company Applicant
Required documents for company application:
1. Copy of business registration and CI
2. HKID copy of the licensee or director of the company
3. Address proof copy of the licensee or director of the company (such as water/electricity/gas or rates bill)

2 本公司代辦申請ESSO埃索油咭為月結黑色油咭。
3 繳款方法： 費用以月結形式支付，無須即時付款，既方便又輕鬆!
月結單會於每月8-10號發出，請於5天或之內繳清付有關款额。
申請費用全免！無手續費，無時段限制，無隱藏收費！
Notes:
1 U-Drive Fuel Card will be mailed to the registered Hong Kong correspondence address of the applicant within 7-10 days.
2 The company is acting as an agent to apply for the ESSO Black Fuel Card.
3 Payment method: Fees can be settled monthly without immediate payment for your ease and convenience.
Monthly statement will be issued on the 8th-10th of each month.
Please settle the relevant amount within 5 business days.
